'A smart guided missile' to cancer cells

A novel therapy aims to treat cancer of the lymph nodes by delivering radiation directly to tumor cells.

The technology, called radioimmunotherapy, involves attaching a radioactive element to a drug designed to latch onto cancer cells. Unlike traditional radiation, which exposes a large section of the body, these drugs shower tumor cells with radiation but leave most other tissues unharmed.

"These are like 'smart' guided missiles that home to where their target is," says Mark Kaminski, a professor at the University of Michigan Cancer Center who helped develop one of the new drugs, Bexxar.

Today, Bexxar and a similar drug, Zevalin, are used mostly as last-ditch treatments for follicular lymphoma, an immune-system cancer. A new study, however, suggests that some patients might want to try these drugs first before undergoing chemotherapy and its grueling side effects.

In the report in today's New England Journal of Medicine, Bexxar shrank tumors in 95% of patients and eliminated them in 75%, says Kaminski, who led the study. Kaminski receives patent royalties on the concept behind the drug, and the study was partly financed by the company that developed Bexxar. Therapy with Bexxar, now sold by GlaxoSmithKline, lasts one week, compared with four to six months of chemo. The new therapies also work as well as chemo but cause fewer side effects.

Follicular lymphoma, which is diagnosed in 15,000 North American adults a year, affects a type of white blood cell, called B-cells, that fights infection. Tumors in lymph nodes grow slowly and sometimes subside after treatment. But the disease never stays away for long and cannot be cured.

Sonali Smith, an assistant professor of hematology and oncology at the University of Chicago, says the new research is promising, but she cautions that the 76 patients in the study may have fared well because they were younger and healthier than average — not because of Bexxar. She suggests that researchers conduct more studies in which they directly compare Bexxar with other options to determine the best sequence of therapies.

Some doctors worry that radiation could put patients at risk for long-term damage to bone marrow, or even leukemia. Still, Michael Williams, director of the hematologic malignancy program at the University of Virginia Health System, says, "If they can go five or seven years without having to deal with further treatment, that's a very positive advance."
